We offer several different health insurance plans to meet your needs. Each plan differs in deductibles, co-insurance, copays, and provider networks.

Preferred Provider Organization (PPO) 90/70

  • Pays 90% of eligible in-network claims
  • Pays 70% of out-of-network claims
  • Your choice of a $500 or $1500 deductible

Preferred Provider Organization (PPO) 80/60

  • Pays 80% of eligible in-network claims
  • Pays 60% of out-of-network claims
  • Your choice of a $500, $1000, $1500, or $2000 deductible

Point of Service (POS) 90/70

  • Pays 90% of eligible in-network claims
  • Pays 70% of out-of-network claims
  • Your choice of a $500 or $1500 deductible

Point of Service (POS) 80/60

  • Pays 80% of eligible in-network claims
  • Pays 60% of out-of-network claims
  • Your choice of a $500, $750, $1000, $1500, $2000, $3000, or $5000 deductible

Point of Service (POS) 70/50

  • Pays 70% of eligible in-network claims
  • Pays 50% of out-of-network claims
  • Your choice of a $1000, $3000, or $5000 deductible

Health Maintenance Organization (HMO) 90

  • Pays 90% of eligible in-network claims
  • No coverage for out-of-network claims
  • No required deductible

Health Maintenance Organization (HMO) 80

  • Pays 80% of eligible in-network claims
  • No coverage for out-of-network claims
  • Individual deductible of $150
  • Family deductible of $450

Qualified High Deductible Health Plans (HDHP)

  • Pays 80% of eligible in-network claims
  • Pays 60% of out-of-network claims
  • Point of Service Network
  • Your choice of $3300 or $4000 plan

Dental Insurance

  • 100% coverage for preventative treatment and diagnostic exams
  • 80% for basic prosthetic treatment
  • 50% for major and restorative work
  • 50% up to a lifetime maximum of $1,000 for orthodontia

Vision Insurance

  • Large network of vision providers including private practice doctors, local optical stores, and national retail stores.
  • $180 allowance on eyeglass frames or contact lenses

Short-Term Disability Coverage

  • Provides a weekly benefit amount to your employees who become totally disabled due to an illness or accidental injury.
  • Members choose the amount of coverage to provide for employees.

Life Insurance Options

In addition to health insurance coverage, the GMEBS Life and Health Insurance Fund offers you the following life insurance coverage:

  • Group Term Life Insurance. You have the flexibility to choose the amount of life insurance coverage offered to employees and dependents.
  • Optional Life InsuranceAllow employees to purchase life insurance in addition to the amount you provide. In order to add this benefit to your city, 30% of employees must elect to participate.
  • Accidental Death and Dismemberment. Covers death or specific types of injury as a result of an accident.
